Completing your forms before your visit helps us get you seen faster. Bring them to the office when you arrive, or upload them through MyChart if you already have portal access.
New patients
The packet is everything in one file — start here.
- New Patient Packet (PDF, 15 pages) — complete before your first visit. It includes your medical, surgical, family and social history, your medications and pharmacy, our office policies, and the privacy forms.
Individual forms
Each of these is a section of the packet on its own, in case you only need one.
- Medical History Form (PDF) — gives your physician a detailed picture of your medical history and current condition.
- Medical Records Release (PDF) — authorizes another provider to send us your medical records.
- Communication Preferences (PDF) — tells us how we may contact you, whether we can leave results, and who we may speak to about your care.
- Office Policies & Procedures (PDF) — our appointment, billing, refill and forms policies, and how visits alternate between your physician and an advanced practice provider.

How do I submit my forms?
Bring the completed forms with you to your appointment. If you already have a MyChart account you can upload them through the portal instead, which is the secure way to send them ahead. New patients are set up with portal access at the first visit, so bringing the packet in is usually simplest. If you would rather email them, scheduling@ganephrology.com will receive them — but ordinary email is not secure, so the portal or the office is the better route.

The New Patient Packet — it contains everything, including the medical history and the forms you’ll sign. Completing it before your visit saves time at check-in.
How do I request my medical records?
