These terms apply to this website. The most important one: what you read here is general education, not medical advice about you.

If you think you are having a medical emergency, stop reading and call 911 or go to the nearest emergency room. Do not use this website, email, or the patient portal to report an urgent problem.

About these terms

This website is operated by Georgia Nephrology, LLC ("Georgia Nephrology," "we," "us"). These terms govern your use of ganephrology.com and everything published on it — the patient-education library, the provider and location pages, and the forms available for download.

By using this site, you agree to these terms. If you don't agree with them, please don't use the site. Being our patient does not depend on accepting these terms, and nothing here changes the care you receive or the rights you have as a patient.

This site is not medical advice

Everything in our condition and treatment library is general health education. It describes how kidney disease and its treatments usually work, for a general audience. It is not a diagnosis, not a treatment recommendation, and not advice about your particular situation — because we haven't examined you, reviewed your history, or seen your labs.

Kidney care is highly individual. Two people with the same diagnosis can need very different treatment depending on their other conditions, their medications, their kidney function, and their circumstances. Never use this website to decide whether to start, stop, or change a treatment or medication. Talk to your physician or another qualified clinician first, and don't delay getting care or disregard advice you've already been given because of something you read here.

Reading these pages does not make you our patient, and it does not create a physician–patient relationship between you and any Georgia Nephrology physician or advanced practice provider. That relationship begins when you are seen and accepted for care by one of our providers, not before.

In an emergency

This website is not monitored, and it is not a way to reach anyone quickly. For a medical emergency, call 911 or go to the nearest emergency room. If you need to reach the practice about an urgent but non-emergency problem, call your office at 404-645-7150 during business hours.

About the information published here

We work to keep this site accurate and current, and our clinical content is reviewed by our physicians before it is published. Even so:

  • Medicine changes. Guidelines, drug approvals, and best practice move on. A page that was accurate when written may not reflect the current standard of care, and we don't promise that every page is up to date at the moment you read it.
  • Practice details change. Providers join and leave, office hours and addresses change, a provider's hospital and dialysis-center coverage changes, and whether someone is accepting new patients changes. Please call to confirm anything you are relying on.
  • Insurance participation changes, and it depends on your specific plan and product — not just the insurer's name. Confirm your coverage with us and with your plan before your visit.
  • Errors happen. If you spot something wrong on this site, we'd genuinely like to know: call 404-645-7150.

Patient forms and downloads

The forms on this site are provided so you can complete them before your visit. They are made available as-is and may be revised; the version our office gives you at the time of your appointment is the one that governs. Downloading or completing a form does not schedule an appointment, register you as a patient, or create any obligation on our part to treat you.

The patient portal and other third-party services

The patient portal (MyChart) is operated by a third party, not by us. Your use of it is governed by that provider's own terms and privacy policy, and these terms stop applying once you leave this website. The same is true of the maps on our location pages and of any other third-party service embedded in or linked from this site.

Links to other websites

We link to hospitals, dialysis providers, patient-advocacy organizations, and other resources because we think they're useful. We don't control them, we're not responsible for their content or their privacy practices, and a link is not an endorsement of everything on the site it points to.

Communications with us

Ordinary email is not secure. Please do not send health information by email — use the patient portal or call us. This is covered more fully in our privacy policy.

Sending us an email, downloading a form, or using this site in any other way does not by itself establish you as a patient, and we can't guarantee that a message sent outside the portal will be seen promptly.

Your privacy

How this website handles information about visitors is set out in our privacy policy. How the practice protects your medical information, and the rights you have over it, is set out in our HIPAA Notice of Privacy Practices — that notice, not this page, is the document that governs your health information.

How you may use this site

You're welcome to read, print, and share the pages here for your own personal, non-commercial use, and to give copies to family members or caregivers helping with your care. Please keep our name and any copyright notice on anything you copy.

What you may not do

  • Use the site for any unlawful purpose, or in a way that could damage, disable, or overburden it.
  • Attempt to gain unauthorized access to any part of the site, its servers, or any connected system — or probe, scan, or test its security without our written permission.
  • Use automated tools to scrape, harvest, or bulk-download the site's content, or to collect provider information for marketing or recruiting.
  • Republish our content commercially, or present it as your own or as coming from someone else.
  • Use our name, logo, or any provider's name or photograph to imply an endorsement or affiliation that doesn't exist.
  • Impersonate anyone, or misrepresent your affiliation with Georgia Nephrology.

Our content, names, and images

The text, design, photographs, and other material on this site are owned by Georgia Nephrology or used with permission, and are protected by copyright and other laws. "Georgia Nephrology" and our logo are our marks. The provider photographs are of real clinicians who work here and may not be reused for any purpose.

If you believe material on this site infringes your copyright, write to us at the address below with enough detail to identify the work and where it appears, and we will look into it.

Anything you send us

This site has no accounts, no comments, and no forms, so there is nothing here for you to post. If you send us feedback or a suggestion by another route — an email about the website, say — you agree we may use it to improve the site without owing you anything for it. This does not apply to your health information, which is protected under HIPAA regardless of how it reaches us.

Trouble using this site

If a page on this site is difficult or impossible for you to use, please call 404-645-7150 and tell us what happened. We'll get you the information another way and work on fixing the page.

No warranties

This website and everything on it are provided "as is" and "as available." To the fullest extent the law allows, we disclaim all warranties, express or implied — including any implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement.

We don't warrant that the site will always be available or uninterrupted, that it will be free of errors, or that it or the servers that deliver it are free of viruses or other harmful components. Some states don't allow certain warranty disclaimers, so parts of this section may not apply to you.

Limitation of liability

To the fullest extent the law allows, Georgia Nephrology and its physicians, providers, employees, and agents are not liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of — or inability to use — this website, including any damages arising from reliance on information published here. This applies even if we've been advised that such damages are possible.

Nothing in this section limits our responsibility for the medical care we actually provide to our patients. That responsibility is governed by the law that applies to medical care in Georgia, and by our professional obligations to you — not by this website's terms. Some states don't allow certain limitations of liability, so parts of this section may not apply to you.

Indemnification

If your misuse of this website — using it unlawfully, breaching these terms, or infringing someone else's rights — causes a claim against us, you agree to defend and indemnify Georgia Nephrology against that claim and the reasonable costs of dealing with it.

Changes to the site and to these terms

We may change, suspend, or discontinue any part of this website at any time. We may also update these terms; when we do, we'll revise the "last updated" date above, and the version posted here is the one that applies. Continuing to use the site after a change means you accept the updated terms.

Governing law

These terms are governed by the laws of the State of Georgia, without regard to its conflict-of-laws rules. Any dispute arising out of this website or these terms will be brought in the state or federal courts located in Gwinnett County, Georgia, and you agree those courts may hear it.

If part of these terms can't be enforced

If any provision here is found unenforceable, that provision is limited or removed to the smallest extent necessary and the rest of these terms stay in force. If we don't enforce a provision on some occasion, that isn't a waiver of our right to enforce it later.
